- Verify your information
- Input your receipt information including Store Number (ST #), Register Number (TE #), Transaction Number (TR #) and Plan Purchase Date
- Input your State of Purchase, Plan Warranty SKU, Product Purchase Price and Plan Purchase Price. Click Identify Your Product
